Payment Information

  • Payment is due at the time of services rendered.
  • All cancellations for scheduled appointments require at least 24 hours notice.
  • All cancellations that are less than 24 hours will be subjected to a cancellation fee.

Insurance Information

  • Dr. Kothari is not contracted with insurance panels at this time.
  • A super bill can be given to the patient for submission to their insurance carrier for reimbursement.
  • The fee for an initial visit is $700; the fee for a follow up visit is $285.
